Description

The Hospital Elder Life Program (HELP) is an innovative delirium-prevention program designed to improve the hospital experience of older patients.
The HELP program aims to:
  • Maintain the cognitive & physical functioning of patients throughout hospitalization
  • Allow patients to be discharged from the hospital as independently as possible
  • Prevent unplanned readmissions.

The HELP team is run by an Elder Life Specialist and a team of volunteers, and offers the following services:

  • Daily visiting, meal support, early mobilization, therapeutic engagement, vision and hearing support, and sleep help.
This is an Out-of-Home Service
Eligibility
Must be an in-patient on specific units at PRHC and at-risk for delirium. Must be at least 65 years of age. Discharge not anticipated within 48 hours, be able to participate in the program, not on droplet or airborne isolation precautions(contact isolation only) and have at least one of the following: Cognitive impairment, changes in mobility, vision/hearing impairment an/or dehydration/malnutrition risk.
Application Process
Referral Required; Referral from a health care or social service provider. Patients can make a referral to this program by speaking to their nurse or allied health provider
