Description

Psychiatric Assessment Services for the Elderly (PASE) at Peterborough Regional Health Centre (PRHC) is a program that offers geriatric mental health assessment, consultation and education services.

  • Individual assessment to address the client's issues through social work, nursing, and/or psychiatric input
  • Education, liaison and support for caregivers, involved professionals and community agencies
  • Psychogeriatric Resource Consultant (PRC) services
  • Recommendation of treatment, services, strategies or case management

The PRC also provides consultation services to employees in the long-term care sector.

Team members can travel to the patient's home or long-term care facility, and satellite clinics are held monthly in Lindsay, Cobourg and Campellford and bi-monthly in Haliburton/Minden. PRC's also take turns being available by telephone for consultation and can be reached toll free at 1-866-877-2910.

Eligibility
Older adults experiencing mental health issues or dementia with behavioural problems. People living in their own home or a long-term care facility, or Caregivers of the above. 65 year(s) and up
Application Process
To access the PASE program, you must be referred by a physician or nurse practitioner. The referral form can be completed and faxed.
